Below you will find downloadable copies of all of the requisite Waukeela forms for you to complete and submit prior to the first day of your daughter’s session at camp.

Registration Form 2011
Complete to enroll your daughter for the coming summer.

Health Form
Complete and have signed by a licensed medical person before your daughter’s session begins.

Travel Form
Complete before your daughter’s session begins to inform us of how and when she’ll arrive.

Community Commitment
Complete and sign with your daughter before her session begins.

Scotch Pine Community Commitment
Campers entering the 10th grade and their parents should complete and sign this form before the session begins.

Campership Application
Complete if your daughter is in need of financial assistance.

Horseback Riding Agreement
Complete if your daughter intends to horseback ride.

Rock Climbing Agreement
Complete if your daughter intends to rock climb. Note there are two forms here - one for camp and one for the International Mountain Climbing School, which provides the equipment and guides.

Whitewater Canoeing Agreement
Complete if your daughter intends to whitewater canoe.
